The dried-out well is Yukio's repressed conscience made literal. He spends the film refusing to look down at the poor he won't treat, then gets dropped to the very bottom of the social order he ignored, forced to claw and scavenge to survive. Only after living as the discarded twin can he recognize that his 'breeding' was always just luck of which infant got kept and which got thrown away.

The brothers' merge is the real horror, not the murder. Costume, camera and class are the only things separating Yukio from Sutekichi, and once Yukio is filthy and feral at the bottom of the well, the visual markers of identity collapse. The snake-scar birthmark that condemned one twin becomes the only proof of who is who, suggesting identity is a costume society assigns, not a thing you are.

Rin's amnesia is a buried verdict against both men. She was Sutekichi's lover from the slums before she became the doctor's wife, and her forgotten past makes her the only witness who knew both twins as equals. Her memory returning is less a romantic reveal than the return of the repressed underclass the film keeps trying to drown.

Meiji-era Tokyo, plague creeping through the slums. Dr. Yukio Daitokuji treats the rich and turns the poor away at his door. Then his parents die, a stench haunts the family estate, and a filthy double climbs out of the dark to pitch Yukio down a dried-out well and slip into his life, his practice, and his amnesiac wife's bed. From the bottom of that pit Yukio learns the truth about the twin cast out at birth over a snake-shaped scar, and about the woman who already knew them both. Tsukamoto trades cyberpunk machinery for class rot and buried blood.
