Kotoko

• The film grew out of Ryukyuan folk-rock singer Cocco, who supplied the original story, the music, and art direction, and took her first starring role as Kotoko. Tsukamoto built the project around her after years of admiring her songs and working with her on music videos and interviews.
• Cocco was prone to self-harm in her youth, and the film deliberately folds her real experience into the character — the wrist-cutting and food fixation read as drawn from life, not invented for shock.
• Tsukamoto self-financed it after struggling to attract investors, shooting digital and handheld and handling most of the below-the-line work himself alongside Cocco. The jittery, unstable camera is engineered to track Kotoko's fracturing perception rather than the room she's standing in.
• Tsukamoto doesn't just direct — he plays Seitaro Tanaka, the celebrated novelist who becomes infatuated with Kotoko and won't be scared off, even after she stabs at him with a fork.
• It premiered at the 68th Venice Film Festival in September 2011 and won the Orizzonti (Horizons) award for best film — the first Japanese feature to take that prize.
• Kotoko self-harms not to die but to confirm she's still real and still allowed to live, and the only relief from the doubles she sees comes when she sings — Cocco's actual instrument doubling as the character's lifeline.

The doubles aren't a gimmick — they're how the film smuggles you inside untreated psychosis without ever naming a diagnosis. Every person split into a gentle version and a violent one externalizes the impossible threat-assessment a sick, isolated mother runs every waking second. The horror isn't that monsters are real; it's that she has no reliable way to tell a monster from a man, so she has to treat everyone as both.

Because Cocco co-authored the story and lived the self-harm, Kotoko plays as a controlled confession routed through a horror director. Tsukamoto casting himself as the novelist who 'discovers' and can't stop watching her isn't an accident — he's literally the outside artist drawn to her pain, half rescuer, half voyeur, and the film knows it. The blurred line between using her story and exploiting it is baked into the casting.

Singing as the only off-switch reframes the whole movie as an argument about what actually keeps a person alive. Medication, romance, and motherhood all fail to stabilize Kotoko; only the act of making music does. The film quietly suggests that for some people creation isn't therapy after the fact — it's the load-bearing wall, and pulling it out is what brings everything down.

A single mother in Tokyo sees everyone twice: one harmless, one ready to hurt her or her infant son. Kotoko can't tell which is real, so she cuts her own arm to prove she still exists. Singing is the only thing that quiets the doubles. When a famous novelist falls for her and refuses to leave, his patience becomes its own kind of pressure. Tsukamoto's camera lurches and stutters with her perception, never letting you settle into who's actually in the room or whether the worst things on screen ever happened at all.
