Riley's final state—conscious but trapped, unable to communicate—mirrors what the entity posing as Mia's mother described during their séance about being trapped and suffering on the other side. The film may be suggesting Mia didn't escape at all but is now experiencing that same trapped consciousness, and the 'rescue' in the hospital was actually her permanent crossing over, not a return.

The ninety-second rule is arbitrary and the spirits know it—they deliberately comply early on to build trust in the game.
