PLOT
- Michele Soavi's second feature as director (after 1987's Stage Fright), produced by Dario Argento, who co-wrote the screenplay with Soavi and Franco Ferrini.
- The film began development as the third installment in the Demons series, following Lamberto Bava's Demons (1985) and Demons 2 (1986), before Soavi reworked it into a standalone story — though the trapped-in-a-locked-building structure survived from that lineage.
- The cathedral scenes were shot primarily at Matthias Church in Budapest and Rome's Church of St. Mary of the Visitation, with additional work at Elios and De Paolis studios; the WWII-ruined shell of St. Nicholas Church in Hamburg was used for the film's final shot.
- Hugh Quarshie — the Ghanaian-British actor later known for Holby City — plays Father Gus, the church's priest, while Tomas Arana plays Evan, the new librarian whose curiosity unseals the crypt.
- Special effects artist Sergio Stivaletti (Demons, Argento's Opera) built the film's signature practical gags, including the goat-headed demon/incubus attack and the climactic demon head assembled from writhing bodies.
