- The film is based on Frank De Felitta's 1978 novel, itself inspired by the real 1974 case of Doris Bither in Culver City, California — she reported being violently assaulted by an invisible presence in her home, a case investigated for weeks by UCLA parapsychologist Barry Taff.
- Stan Winston and effects artist James Kagel built the practical rigs for the assault scenes, including a latex dummy body wired with internal suction cups so the crew could simulate unseen fingers pressing into flesh — the dummy alone reportedly cost $65,000 to build.
- David Labiosa, who played Carla's teenage son, really broke his wrist filming a scene where his character gets thrown while trying to protect her — an eerie echo of the real Doris Bither case, where her actual son broke his arm in a nearly identical moment.
- Cinematographer Stephen H. Burum shot the film with extreme canted 'Dutch' angles and heavy use of the split-diopter lens, keeping both foreground and background in sharp, disorienting focus so the camera itself feels like an unseen presence in the room.
