- Rejected for a UK theatrical certificate in 1984, with chief censor James Ferman ordering the print re-exported back to Italy rather than risk prosecution. The film stayed off UK shelves until 2002, when distributor Vipco finally secured a certificate cut by 22 seconds to soften the razor-blade murder scene; Shameless's 2007 DVD and 2011 Blu-ray gradually restored most of the missing footage.
- Fulci gives himself a cameo as the Chief of Police, ordering Lt. Williams to keep the Ripper's methods out of the press so the city doesn't panic.
- Shot almost entirely on real New York City locations, unusual for an Italian giallo of the era — including Times Square/42nd Street at the peak of its grindhouse-and-porn-theater period, a subway sequence at Brooklyn's Hoyt-Schermerhorn station, and 'studio' interiors built inside the Kingsbridge Armory in the Bronx.
- Special make-up effects artist Maurizio Trani handled the film's most notorious gore, including the razor-blade murder that specifically drew the BBFC's censorship attention.
- Reception split hard on release: critic Alan Jones called it a 'psychotic, erotic masterpiece' in 1983, while others attacked it for misogyny (genre critic Chas. Balun later wrote it off as 'justifiably overlooked'). It holds just a 25% Rotten Tomatoes score today but has built a real cult following since.
