Anna and Alfredo are the same person from the start. Every scene of Alfredo stalking her is Anna stalking herself, and the 'recovery' in the second act is just the host personality forgetting what the other one does at night.

The film is Argento's confession that the male gaze he built his career on is itself a pathology. The camera that undressed Suzy Bannion and Jennifer Corvino comes home to roost—it now turns on his own daughter, and the violence it inflicts is no longer deniable as 'style.' The blood-painting sequence is the director admitting his art has always been made from women's suffering.

Stendhal Syndrome functions as a literal metaphor for horror fandom: the inability to distinguish art from reality, the compulsion to consume beauty until it destroys you. Anna's descent isn't into madness but into the condition every Argento superfan already inhabits—finding pleasure in images that should repulse, and losing the boundary between spectator and participant.

Detective Anna Manni chases a serial rapist-killer through Florence and walks straight into a psychosomatic trap: Stendhal Syndrome, where Renaissance beauty literally swallows her whole. One painting vomits her into its waters; another dissolves her into screaming fish. The rapist Alfredo catches her, brutalizes her, and vanishes. She retreats to her father's house in her hometown of Viterbo, but the syndrome metastasizes—she starts painting with blood, shredding photographs, forgetting her own face. When Alfredo resurfaces, Anna kills him. Then keeps killing. The final twist: she was always the thing hunting her. Argento's most self-lacerating film turns the gaze inward and finds a monster staring back.
